Review of Systems
General
: No fevers and chills. No weight loss.
Skin
: Noncontributory
HEENT
: No nasal congestion or throat pain.
Pulmonary
: No cough. No hemoptysis. Dyspnea recently but only with the chest pain.
Cardiovascular
: No orthopnea, paroxysmal nocturnal dyspnea, lower-extremity edema, light-headedness, or palpitations.
Gastrointestinal
: Nausea during the chest pain episodes. No vomiting.
Genitourinary
: Noncontributory
Musculoskeletal
: She has no muscle aches, joint pain, or stiffness.
Neurologic
: No syncope. No progressive weakness or loss of sensation.
Psychiatric
